The Cook mausoleum is across the road from the similar Blain mausoleum, offset perhaps thirty feet to one side. The design is nearly identical, save that the Cook mausoleum has its front window frame filled with the same sort of stones that make up the mausoleum walls (rather than a marble panel carved with names as the Blain mausoleum has), and has rooftop ornaments that the Blain mausoleum lacks: four urns at the corners, a chequred box at the summit, and a granite ball atop that.

On the round inset panel over the door is the family name, Cook, and the date 1880; both these mausoleums were apparently constructed in this same year.
